There are 42 Community Care Access Centres (CCACs) in Ontario, two of which are hospital-based. CCACs provide a simplified point of access to long-term care for more than 400,000 people each year.
- CCACs arrange and authorize visiting health and personal support services in peoples' homes;
- authorize services for special needs children in schools;
- authorize admissions to long-term care homes;
- provide information and referrals to the public about other community agencies and services.
